WAIVER/RELEASE
I, _________________________________________________________,
in consideration of permitting me to participate in the EagleviewSM Corporate Center Softball League (called the “League”),
and fully desiring to so participate, hereby agree to release EagleviewSM Recreational Committee, EagleviewSM Corporate Center
Association, The Hankin Group, Hankin Enterprises, Hankin Properties Partnership and each of their successors, assigns, members,
affiliates, agents, officers, and employees (collectively called “Sponsors”) and the members affiliates, and officers
of, and participants in the league (collectively called the “League Members”) from any and all claims of any kind
and nature, including, but not limited to, damages for bodily injury to me or to others, either direct or consequential, which
may arise from my participation in any function sponsored by the “Sponsors” and/or “League Members”
including, but not limited to, softball games, or other sports activities.
Consumption of alcoholic beverages on
the premises while participating in any function sponsored by the “Sponsors” and/or “League Members”
is strictly prohibited.
